ALL families must fill out the google form with their student's emergency health information. This MUST be filled out the first week of school. Of note: it is different from the contact information form that is a district form for Aspen.
If your student has any medications that need to be dropped off such as daily meds to be taken in school, inhalers, epipens or other emergency medications, please have them drop them off to us the first week of school. Here are the forms that you will need:
Medication Form
Asthma Action Plan
Allergy Anaphylaxis Plan
If you have any questions, please let us know. Our number is 508-242-8568. Our fax number is 508-359-4904.
