Sixth-Eighth Grade Health Information
Parents/guardians are encouraged to send in a copy of your child's physical every year following his/her annual physical to keep your child's school health record updated.  If your child will be participating on a school sport's team, then a current physical within thirteen months of the first day of practice for that sport is required to be on file in the school nurse's office.  Please contact our school nurse, Tricia Williams, if you have any questions or concerns.

Seventh Grade Health Information
Effective September 1, 2011 all students entering 7th grade must have received the following immunizations in order to attend public school as mandated by the Massachusetts Department of Public Health:

1 booster dose of tetanus (Tdap or Td)
2 doses of measles, mumps, rubella (MMR)
3 doses of Hepatitis B
3 doses of Polio
2 doses of varicella vaccine (Varivax), unless there is a physician certified history of chickenpox disease

Only medical exemptions certified by a physician or documented religious exemptions are allowed.  In order to comply with the above noted public health regulations, please send in a copy of your child's physical prior to the first day of school if you have not already sent in a copy of his/her most recent physical over the past year.  Physicals were requested with the Nature's Classroom paperwork in the early spring; however, many students had physicals scheduled later in the spring or summer and these updated physicals should be mailed to Blake Middle School to the attention of our school nurse, Tricia Williams, as soon as possible.
